| Our major task is to perform quality assurance and certification inspection to ensure that your helidecks intallation for helicopter operations to helidecks and shipboard heliports meet the minimum requirement stated on the current regulations. Routine helideck inspections are usually conducted on an annual basis (dependant on type of installation), while a major inspection of the helideck (design and systems) is performed every 5(five) years. Our 'acceptance' will later forward to Directorate General Petroleum and Natural Gas (MIGAS) for an 'approval'. |

| Background: Between 1976 and 2002 seven fatal accidents claimed the lives of 88 offshore workers and flight crew whilst travelling in offshore helicopters. Catastrophic component failure was the primary cause of 4 accidents and the other 3 were attributed to human factors. There have also been two HLO fatalities on helidecks. Occasionally, reportable (non-fatal) accidents have also occurred, just as they do in other aviation sectors. These have included lightning strikes, major airframe damage, loss of engine power, tail rotor damage and loss of flight control. In most of these cases only the helicopter has been damaged but, infrequently, these incidents have resulted in injury to personnel. |

| Recent research has clearly shown the need for improvement in particular aspects of the design and operation of helidecks on offshore installations for the safety of offshore helicopter flight operations. New designs and existing installation of helidecks need to take this research into account. |

| In December 1995 a Sikorsky S61 helicopter, on final approach to the Claymore 'A' Accommodation Platform (CAP), became engulfed in hot plumes from gas turbine exhausts on an adjacent production platform (CPP). As a consequence, the pilot was unable to sufficiently arrest the helicopter's rate of descent for a safe landing, and the aircraft heavily impacted the CAP helideck, sustaining major damage. This offshore helideck incident exposed the 2 flight crew and 14 passengers to extreme danger. |
